Digital packets are available below. Paper packets will be available on and before registration day.

Parents may drop off completed packets at the office at any time prior to August 7th.

Please fill out the following forms and return to the office:

Returning Student Pack - fill out once per family

Health Forms - fill out for each student in family

Free and Reduced Lunch forms and information will be available after August 1st. Free and Reduced Lunch forms must be physically turned in and have an ink signature. No email forms will be accepted. 2023-2024 Free & Reduced Meals Applications

You may submit these forms one of two ways:

• Submit completed forms via email to ndavis@cotopaxire3.org

  • Please note email - while a convenient means of transmitting documents - may not provide the desired level of security for transmitting sensitive or confidential information

• Print completed forms and hand-deliver to the front office starting July 31st

  • The front office will be open from 8 AM to 2 PM, Monday-Thursday

If your family has any contact or information changes from the last school year, please notify the front office of these changes when you submit your forms.

• Copy of Birth Certificate

• Immunizations or Statement of Exemptions (please see Nurse's Corner)

• Kindergarten students must be 5 before August 1st of the school year.

**All forms must be filled out and signed and all required documents received before students may start school.**

Out of district students (parents) must write a letter to the School Board stating the reason for wanting to attend Cotopaxi School District.
