The Colorado Department of Public Health and Environment (CDPHE) will be hosting a vaccination clinic this Thursday, February 25th from 3-6PM at the Cotopaxi School.
Currently, individuals eligible for this clinic include those over the age of 65 and Cotopaxi School staff.

Vaccines will be administered in the school cafeteria. Please fill out and bring the Vaccination Administration and Screening Form at the scheduled appointment time.
There are only a few spots open, and due to demand, those wishing to receive the vaccine should sign up as soon as possible, first come, first serve.
